You wake up, get ready for the day and begin your usual activities. Yet, as the morning progresses, your eyelids feel heavy. You struggle to concentrate, your mind seems less alert, and you find yourself fighting the urge to sleep. During a meeting, you catch yourself nodding off. You wonder why this is happening when you believe you have had enough sleep. This daytime sleepiness can be really bothersome.

If you are living with hypothyroidism, you may naturally suspect your thyroid. After all, you have probably heard that an underactive thyroid can make you feel tired and sluggish.

But is your thyroid the only possible explanation?

Let us meet John, a working professional who finds himself facing precisely this problem.

Meet John: Eight Hours of Sleep, Yet Still Sleepy

John has hypothyroidism. He takes his prescribed thyroid medication and attends his medical appointments. He generally spends about eight hours in bed every night and believes that he is doing what he should to look after his health.

Nevertheless, he struggles to remain alert during the day. He finds it difficult to concentrate at work, and sometimes his eyelids become so heavy that he can barely keep them open.

John begins to wonder:

“If I am sleeping for eight hours, why do I still feel so sleepy during the day? Could my thyroid be responsible? Or is something else disturbing my sleep without my realising it?”

These are sensible questions. The answers matter because daytime sleepiness is not always caused by hypothyroidism, even in someone who has been diagnosed with the condition.

Before we explore John’s problem, let us understand an important distinction.

Fatigue and Sleepiness: Are They the Same?

People often use the words tiredness, fatigue and sleepiness as though they mean the same thing. Although they can occur together, they are not identical.

Fatigue is a feeling of exhaustion, reduced energy or difficulty carrying out ordinary activities. You may feel tired even when you are not sleepy enough to fall asleep.

Daytime sleepiness, on the other hand, is a tendency to fall asleep or struggle to remain awake when you are expected to be alert.

A person with hypothyroidism may experience fatigue, sluggishness and difficulty concentrating. If thyroid hormone levels are not adequately controlled, these symptoms may persist.

However, repeatedly nodding off during the day raises another question: Is the person getting refreshing sleep at night?

Eight hours in bed do not necessarily mean eight hours of good-quality sleep. Repeated interruptions may leave a person inadequately rested even when they are unaware of waking up.

Let us look at the possible reasons.

Why Might John Have Daytime Sleepiness Despite Eight Hours of Sleep?

Several factors may contribute to John’s symptoms. They can occur separately or overlap with one another.

1. Slower Body Processes and Reduced Energy

Thyroid hormones help regulate the body’s energy use and many of its essential functions.

When hypothyroidism is inadequately treated, the body’s processes may slow down. A person may feel sluggish, lack energy and find ordinary activities more demanding.

However, fatigue caused by hypothyroidism is not necessarily the same as a tendency to fall asleep during the day. If John continues to feel sleepy despite satisfactory thyroid test results, it is important to look for other explanations rather than automatically increasing or changing his thyroid medication.

2. Brain Fog: When Thinking Becomes Difficult

Some people with hypothyroidism describe a feeling of brain fog.

They may find it difficult to concentrate, remember details, organise their thoughts or work at their usual speed. They may feel mentally clouded even when they are trying their best to remain attentive.

Poor-quality sleep can also impair concentration and mental alertness. As a result, brain fog and daytime sleepiness may occur together, making it difficult for a person to identify the underlying problem.

3. Obstructive Sleep Apnoea: A Hidden Disturbance During Sleep

One important possibility is obstructive sleep apnoea, commonly abbreviated as OSA.

Apnoea means a temporary stopping of breathing. In sleep apnoea, breathing repeatedly stops or becomes significantly reduced during sleep.

In the obstructive form, the upper airway—the passage carrying air from the nose and mouth towards the lungs—repeatedly narrows or closes during sleep.

Although the person may appear to be sleeping, these interruptions can disturb the normal continuity of sleep. The brain may briefly arouse the person to restore breathing, sometimes without the person remembering having awakened.

This can happen repeatedly throughout the night.

The result may be unrefreshing sleep, morning tiredness, difficulty concentrating and excessive sleepiness during the day.

4. Disturbed Sleep Patterns

Sometimes the problem is not the number of hours spent in bed but the quality and timing of sleep.

Repeated awakenings, irregular sleeping hours, certain medicines, discomfort, other sleep disorders or environmental disturbances may prevent a person from obtaining refreshing sleep.

Someone may believe that they have slept continuously for eight hours when, in reality, their sleep has been interrupted several times.

If John is experiencing this problem, simply going to bed earlier may not be enough. The cause of the disturbed sleep needs to be understood.

5. Changes in Hunger, Satiety and Body Weight

Sleep and appetite are connected in ways that are more complex than many people realise.

Poor sleep may influence hunger, food choices and the way a person responds to feelings of fullness. In some people, these changes may contribute to increased food intake or difficulty maintaining healthy routines.

Weight is influenced by many factors, including diet, physical activity, medicines, sleep, stress and underlying health conditions. Poor sleep does not inevitably cause weight gain, but it may be one contributing factor.

Excess body weight can, in turn, increase the risk of obstructive sleep apnoea in some people, creating a possible connection between these problems.

To understand how these factors might interact, let us return to John.

How Can Poor Sleep Affect the Body Beyond the Bedroom?

Sleep is not simply a period during which the body switches off. During healthy sleep, the brain and body carry out important processes that support attention, emotional regulation, metabolism and general well-being.

When sleep is repeatedly interrupted, several of these processes may be affected.

The Body’s Stress Response

Our bodies have a built-in system for responding to challenges. Hormones, including cortisol, help regulate this response and follow a normal daily rhythm.

Repeated sleep disruption may influence the body’s stress-regulation systems. However, this does not mean that everyone who sleeps poorly will have persistently elevated cortisol levels.

Think of the body as a house with an alarm system. The alarm is useful when there is a genuine threat. But if it keeps sounding through the night, the occupants cannot rest properly, even if they do not remember every interruption.

Similarly, disturbed sleep may leave a person feeling unrefreshed and less able to manage the demands of the following day.

Two Messengers: One Associated with Hunger, the Other with Satiety

Two hormones often discussed in relation to appetite are ghrelin and leptin.

Ghrelin contributes to feelings of hunger and commonly rises before meals. Leptin, produced mainly by fat cells, helps inform the brain about the body’s energy stores and participates in the longer-term regulation of appetite and energy balance.

Sleep can influence appetite and food-related behaviour. Research suggests that insufficient or disrupted sleep may affect hunger, food choices and food intake, although the effects on individual hormones are not identical in every person.

Imagine John finishing dinner and feeling comfortably full. Later in the evening, he finds himself looking for something else to eat.

It would be incorrect to assume that this behaviour proves his ghrelin or leptin levels are abnormal. Nor would it be fair to conclude that he simply lacks willpower.

Appetite is influenced by several interacting factors, including sleep, stress, habits, emotions, food availability, medicines and individual physiology.

Understanding these influences may help John approach his difficulties more constructively.

How Might Weight Gain and Sleep Apnoea Reinforce Each Other?

Suppose John’s sleep remains disturbed for months. If poor sleep contributes to changes in appetite, food choices or daily activity, these changes may combine with other factors and contribute to weight gain.

If he gains excess weight, the tissues around his neck and upper airway may increase in some circumstances, making the airway more likely to narrow during sleep. This may increase the risk or severity of obstructive sleep apnoea.

Sleep apnoea can then interrupt sleep repeatedly, leaving John sleepy during the day. Reduced energy and poor alertness may make it more difficult for him to maintain his usual activities and healthy routines.

If sleep-related changes in appetite continue, the difficulties may reinforce one another.

This creates a possible vicious cycle:

Disturbed sleep may contribute to changes in appetite and daily routines.

These changes, along with other factors, may contribute to weight gain.

Excess weight may increase the risk or severity of obstructive sleep apnoea in some people.

Sleep apnoea may further disrupt sleep and worsen daytime sleepiness.

This cycle is possible, not inevitable. Weight gain has many causes, and obstructive sleep apnoea can occur in people who are not overweight.

The important point is that identifying and treating a sleep disorder may address one significant part of the problem.

What Might John’s Wife Notice at Night?

People with obstructive sleep apnoea are not always aware that their breathing is being interrupted. Sometimes, it is a bed partner who first notices something unusual.

John’s wife may observe loud snoring, pauses in breathing, or episodes in which he suddenly gasps, snorts or resumes loud breathing.

Why does this happen?

When the upper airway narrows, John must work harder to draw air through the restricted passage. The movement of air causes soft tissues in the airway to vibrate, producing the familiar sound of snoring.

If the airway becomes completely blocked for a period, airflow may stop despite continued efforts to breathe. When the airway opens again, John may gasp, snort or suddenly resume loud snoring.

His breathing may become quieter if he changes position—for example, by turning onto his side. In some people, this allows the airway to open more fully for a time. However, the effect varies, and breathing interruptions may return.

John may not remember any of these events the following morning. His wife’s observations could therefore provide valuable information for his doctor.

Loud snoring accompanied by witnessed pauses in breathing, gasping or persistent daytime sleepiness deserves medical assessment. Snoring alone does not establish a diagnosis of sleep apnoea.

When John Finally Realises That Something Must Be Done

For some time, John continues with his usual routine. He does not pay much attention to his daytime sleepiness. He assumes that he is tired from work and that a little extra rest might solve the problem.

Meanwhile, his sleep may continue to be disturbed, and his weight gradually increases. He does not fully appreciate how these problems might be connected or how one may be making another worse. Without recognising what is happening, he carries on much as before.

Then, one day, an incident changes everything.

John is sitting behind the steering wheel of his car at a traffic signal. He is waiting for the light to change when his eyes begin to close. He briefly dozes off.

The signal turns green, but John does not move. His wife, sitting beside him, realises that something is wrong and shakes him awake. Startled, he regains awareness and manages to move the car forward.

The incident frightens him. Until that moment, he had thought of his daytime sleepiness as an irritating problem that he could manage. Now he realises that it could have serious consequences—not only for him but also for his wife and other road users.

John decides that he can no longer ignore the problem. He needs to speak to his doctor and understand why he is struggling to stay awake despite spending what seems to be enough time in bed.

This is an important lesson for all of us: persistent daytime sleepiness is not something to dismiss, particularly when it interferes with driving, work or other activities that require alertness.

Anyone who becomes drowsy while driving should stop in a safe place and must not continue driving while sleepy. Being awakened does not mean that it is safe to resume the journey.

How Can John Break This Vicious Cycle?

John’s experience at the traffic signal is a warning that he must not ignore. He needs medical advice rather than assuming that his sleepiness is simply another symptom of hypothyroidism.

His doctor can review his thyroid treatment, sleep habits, symptoms, medicines and other possible causes of daytime sleepiness.

If obstructive sleep apnoea is suspected, the doctor may recommend a sleep study.

A Sleep Study: A Simple Step Towards Finding the Answer

A sleep study is generally a straightforward, non-invasive investigation. Depending on the type recommended, John may be able to sleep in his own bed while using a small monitoring device, or he may spend a night at a sleep centre.

The equipment records relevant information about his sleep and breathing. Depending on the study, this may include airflow, breathing effort, oxygen levels and heart rate, along with other measurements.

The procedure is generally painless, although the monitoring equipment may take some getting used to. John does not have to perform any special task; he simply follows the instructions given to him.

The results help the doctor determine whether sleep apnoea is present and guide the choice of treatment. Not everyone requires the same type of sleep study, so the doctor will recommend the appropriate investigation.

The Good News: Effective Treatment Is Available

Suppose John’s sleep study confirms obstructive sleep apnoea. What happens next?

One commonly used and effective treatment is CPAP (Continuous Positive Airway Pressure).

John wears a mask over his nose, or over his nose and mouth, while sleeping. A small machine delivers a gentle flow of pressurised air through the mask. This pressure helps keep his upper airway open and prevents repeated interruptions in breathing.

In simple terms, the device helps maintain a free passage for air while John sleeps.

With appropriate treatment and regular use, many people experience better-quality sleep, less snoring and a substantial reduction in daytime sleepiness. John may find it easier to remain alert, concentrate on his work and manage his everyday activities.

Other approaches may be suitable for some people, depending on the nature and severity of their condition. His doctor can help him choose the most appropriate treatment and follow up to ensure that it is working well.

Can John Finally Escape the Vicious Cycle?

Let us return to the cycle we have been discussing.

If sleep apnoea repeatedly interrupts John’s sleep, he may remain sleepy during the day. Poor sleep may also influence his appetite, food choices and energy levels. If these changes contribute to weight gain, excess weight may further increase the risk or severity of obstructive sleep apnoea in some people.

Effective treatment can interrupt an important part of this cycle by improving breathing during sleep and reducing sleep disruption.

If John also needs help managing his weight, feeling better rested may make it easier for him to work towards healthier routines. However, weight changes are not guaranteed simply by treating sleep apnoea, and additional measures may be needed.

The message is encouraging: this cycle is not inevitable, and meaningful improvement is a real possibility.

With proper assessment, appropriate treatment and follow-up, many people with obstructive sleep apnoea experience meaningful relief from their symptoms and a better quality of life.

We are not promising that every symptom will disappear or that weight will automatically fall. We are recognising something equally important: the problem may be treatable, and improvement may be within reach.

A Final Word of Encouragement

If you recognise yourself in John’s story, please do not lose hope or assume that you must simply learn to live with the problem.

The first step is to understand what is causing your symptoms. With appropriate assessment and treatment, many people can sleep better, feel more alert and regain confidence in their everyday lives.

Sometimes, the way forward begins not with changing your thyroid treatment, but with discovering what is happening while you sleep.

Key Take-Home Messages

1. Eight hours in bed does not always mean eight hours of refreshing sleep.
The quality of sleep matters as much as the time spent in bed.

2. Fatigue and sleepiness are not the same.
Hypothyroidism may contribute to tiredness and brain fog, but persistent difficulty staying awake deserves further assessment.

3. Do not automatically blame every symptom on hypothyroidism.
Even when thyroid blood tests are satisfactory, other conditions may be responsible for continuing symptoms.

4. Obstructive sleep apnoea is an important, treatable possibility.
Loud snoring, witnessed pauses in breathing, gasping during sleep and persistent daytime sleepiness are reasons to seek medical advice.

5. A sleep study can help identify the problem.
The appropriate investigation is generally straightforward and non-invasive, and some people can undergo it at home.

6. Effective treatment can make a real difference.
For suitable patients with obstructive sleep apnoea, CPAP helps keep the airway open during sleep. Many experience better sleep and reduced daytime sleepiness with appropriate treatment.

7. The vicious cycle can be interrupted.
Better sleep may improve daytime alertness and make healthy daily routines easier to maintain. Improvement is a genuine possibility, although individual results vary and weight loss is not guaranteed.

8. Never ignore sleepiness while driving.
If you feel drowsy at the wheel, stop in a safe place and do not resume driving while sleepy. Seek medical assessment for persistent daytime sleepiness.

About the Author

Dr. Man Mohan Varma, MS (Surgery) is a consultant surgeon with more than 45 years of clinical, academic, and surgical experience. His goal is to help readers better understand their health, participate more confidently in discussions with their doctors, and make informed decisions about their care.

Educational Disclaimer

This article is intended for educational purposes only and should not be considered a substitute for professional medical advice, diagnosis, or treatment.

The information provided here is general in nature and may not apply to every individual situation. Every patient’s circumstances are unique. Decisions regarding symptoms, investigations, diagnosis, treatment, medications, lifestyle changes, and follow-up should always be made in consultation with a qualified healthcare professional who is familiar with your medical history and circumstances.

End of Part 9 of a 14-Part Series: Living Well with Hypothyroidism

Coming Next:

Why Has My Menstrual Cycle Changed?

Have your periods become irregular, unusually heavy or lighter than before? Have the intervals between periods changed, or have your periods stopped for several months? These changes can be confusing and sometimes worrying, particularly when you are already living with hypothyroidism.

Thyroid hormones play an important role in the normal functioning of the reproductive system. When thyroid function is disturbed, menstrual patterns may sometimes change. However, the thyroid is not the only possible explanation. Other hormonal influences, age-related changes and gynaecological conditions may also contribute.

If you have a history of polycystic ovary syndrome (PCOS), you may have experienced irregular periods even before your thyroid problems were diagnosed. Understanding how these conditions may overlap—and why one should not automatically assume that every menstrual change is caused by the thyroid—is an important part of looking after your health.

In Part 10, we will explore the possible relationship between hypothyroidism and menstrual changes, consider the role of PCOS, and discuss when it is sensible to seek medical advice. Our aim will be to help you understand your body’s signals without unnecessary anxiety and recognise when further assessment may be helpful.

That is where we shall begin our next conversation.

Understanding Why Daytime Sleepiness May Persist Even When You Seem to Be Getting Enough Sleep Part 9 of a 14-Part […]

"_jetpack_memberships_contains_paywalled_content":false
_jetpack_memberships_contains_paid_content":false
[8
96,92,91,93
post-347","post","type-post","status-publish","format-standard","has-post-thumbnail","hentry","category-thyroid-weight","tag-cpap-therapy","tag-daytime-sleepiness","tag-obstructive-sleep-apnoea","tag-sleep-apnoea-symptoms

Daytime Sleepiness Despite 8 Hours of Sleep: Why?

<meta name="description" content="Daytime sleepiness despite eight hours of sleep?
